YOU CAN POST YOUR PRESCRIPTION TO US!

• Request a new prescription as usual from your GP surgery
  WE HOLD STOCK OF ALL MANUFACTURERS PRODUCTS
• Collect your prescription, sign it on the back and tick the correct box if you are exempt from charges
• A REGISTRATION FORM is available from your Local Service Centre.
• Once you have completed the registration form send it along with your signed prescription in our Freepost envelope. We will call you to arrange a suitable delivery day when we receive the prescription.
